Introduction

The Confederation, Hillingdon CIC, aimed to enhance the quality of information provided to Primary Care Networks (PCNs) in the borough. They engaged Primary Care IT to assist practices in meeting their Enhanced Services and IIF targets for the PCN DES, leveraging its specialized industry knowledge of EMIS and broader systems integration. The project faced significant challenges due to complex business rules, requiring extensive support to practices, particularly for Diabetes Enhanced Service delivery. Primary Care IT addressed these challenges through various services, including setting up practice alerts and providing templates within clinical systems to identify patients needing immediate action. Innovative practice dashboards on the OneAnalytics platform facilitated progress monitoring, highlighted potential income, and supported decision-making on resource allocation.

Partnership Objectives:

  • Provide timely, accurate, and complete Diabetes business intelligence for 2022/23.
  • Establish a single-user web portal for clinicians and non-clinicians with relevant Diabetes information accessible 24/7.
  • Ensure data quality assurance for coding and templates according to complex NHS England business rules standards.
  • Install user-friendly resources and tools, particularly Diabetes Enhanced Service templates, for PCNs and practices to optimize patient care.
  • Offer education and training operational throughout the year.
  • Assist PCNs in achieving value for public funds for 2022/23.

Solution

Primary Care IT implemented a suite of services to address the project’s challenges:

  • Setting up practice alerts and providing templates within clinical systems.
  • Developing innovative practice dashboards on the OneAnalytics platform.
  • Providing facilitators to assist PCNs in platform navigation and plan development.

Outcome

The project yielded several significant benefits:

  • Reduced performance reporting time by an average of 4 weeks, enabling earlier decision-making.
  • Hillingdon PCNs attained additional income of £145,542 compared to NW London ICS practices.
  • Each practice achieved 90% of diabetes targets, surpassing NW London ICS practices’ performance (77%), contributing to broader ICS objectives for Diabetes and NDS.
  • High provider satisfaction against contract requirements in 2022/23.
  • Improved planning and resource allocation through practice-by-practice performance reports forecasting end-of-year positions.
  • Enhanced performance potentially saving the NHS £453,3111 and gaining diabetics in Hillingdon an additional 656 years of healthy life.
  • Potential savings of £3,718,346 and an additional 5379 years of healthy life across the ICS if similar improvements are replicated.
