Updated QOF Tools for 2026/27 (EMIS Resources)

📢 We’re releasing this week a set of updates to support practices with the latest QOF 2026/27 requirements across our OneTemplates, OneMonitoring and Handy Protocols. These updates are designed to help practices identify outstanding QOF activity earlier, improve coding accuracy at the point of care, and support clinical teams with new and updated indicator requirements across asthma, COPD, NDH/GDM, cardiovascular disease, obesity, hypertension, diabetes, frailty and heart failure.

What’s Included?

OneTemplate updates
All QOF-related areas within our templates have been reviewed and updated to align with the latest QOF 2026/27 indicator changes. The release includes updated template content for asthma, COPD, NDH/GDM, cardiovascular disease, CHD, stroke/TIA, obesity, hypertension, diabetes and heart failure.
This includes support for updated or new indicators including: AST014, AST015, COPD010, NDH003, CD001, CD002, OB004, OB005, HYP010, HYP011, DM037 and HF009.
Key improvements include refreshed asthma age eligibility, updated COPD register logic, gestational diabetes pathways within NDH, revised blood pressure guidance including frailty handling, new obesity indicator support, DM037 8 Key Care Processes support, and HF009 four-pillar therapy review for patients with HFrEF.

OneMonitoring updates
OneMonitoring has also been updated to align with the latest QOF logic. This release includes refreshed register logic, updated frailty handling, new obesity requirements, DM037 monitoring, heart failure updates and changes across several indicator pathways.

OneMonitoring now gives clearer visibility of patients who may need review, follow-up or coding updates, including patients on the heart failure register who do not currently have reduced ejection fraction recorded. It also includes updated logic for “due now”, “out of range”, “just needs doing” and “due by year end” prompts.

New Handy Protocols
We’re also releasing five Handy Protocols to support QOF optimisation and coding accuracy during routine clinical workflows. These include:

🔵 HP345 – Frailty Status and Assessment
Helps users identify where frailty coding may affect QOF blood pressure or diabetes achievement.

🔵 HP449 – Obesity Referral and Eligibility Checker
Supports the new OB004 and OB005 indicators by prompting users when BMI entry may require weight management referral, obesity pharmacotherapy review or further eligibility checking.

🔵 HP450 – COPD Register Review Alert
Highlights patients who appear to have been added to or removed from the COPD register following updated QOF COPD register rules.

🔵 HP451 – Heart Failure Subtype Review Alert
Supports review of patients on the heart failure register who do not currently appear to have HFrEF recorded.

🔵 HP452 – Frailty QOF Coding Alignment Alert
Helps align Rockwood frailty recording with QOF-counting frailty coding when frailty status is recorded during a consultation.
