Thank you for taking the time to read and we hope you find this week’s tips useful!

AccuRx

Our development team doesn’t just build resources for use in the clinical system. We apply our extensive knowledge of the clinical systems and contractual business rules to identifying problems and pushing for change. This was evident last year where we highlighted loopholes and errors about IIF indicators, and offered alternative solutions to NHS Digital, EMIS, and TPP.

This work continues in 2024; AccuRx users may have read an email this week which advised of changes to coding for the blood pressure Florey.

We have supported AccuRx’s clinical team to understand why the previous choice of codes was not working for EMIS Practices. Both organisations are continuing to press EMIS to amend their blood pressure QOF indicators which have been made harder to achieve than in other clinical systems.

EMIS subscribers have access to searches and reports in folder 004B which will help to identify blood pressures below target that have not been counted for QOF indicators.

IIF: ACC-08 / GPAD

We are continuing to develop resources to support Practices working towards the ACC-08 indicator. This is an indicator which requires a whole-team effort all year in order to reach the ambitious target.

Practices should be aware that the PCA (exception) flags available when booking appointments are still in an evaluation phase, and we do not expect them to remove appointments from the overall achievement for 2023/24.

We have spoken to some Practices in the past few weeks who wanted to understand why their GPAD performance was worse than they expected. There are many reasons why achievement may not be where you think it is, and often an understanding of how the Practice’s appointment book works is needed to explain what is happening.

  • New users being given the Default slot type. Default is usually mapped to General Consultation Routine which counts towards the 14 day measure.
  • Nursing staff using slot types mapped to categories counting towards ACC-08. Most Practice nursing (and HCA) activity is planned and falls outside of the scope of ACC-08. Try to make sure that this activity uses different slot types to those used by GPs and other clinical staff seeing ‘acute’ cases.
  • Changing slot types after the appointment time does not improve achievement. Changes to slot mapping are only effective on appointments that have not happened.
  • Untimed appointments do not appear to be counted towards ACC-08. We are still investigating to confirm if this is the case everywhere, but it helps to explain differences where Practices have built their own reports to track achievement.

At this late stage in the year it may be too late to improve achievement for ACC-08, but this is a perfect time to ensure that everything is in place for 2024/25 to maximise achievement if the indicator continues in its current form.

EMIS subscribers can request some reports for the Appointment Reporting module to show month by month achievement, and to help identify mapping issues that are reducing achievement.

TPP does not offer a front end reporting function to track appointment mapping.

Have a patient needing regular home visits?

For SystmOne practices, If you have a patient that is needing regular home visits then you can set up recurring visits from within the home visits booking screen. This means that you will help prevent missed visits as you are able to stipulate a date you want them to apply until.
